Under eye hollows develop through a combination of factors that are largely independent of lifestyle. Volume loss in the mid-face, including the fat pads beneath the eye and through the cheek, creates a shadow in the tear trough that reads as darkness even when no true pigmentation is present.
Bone resorption through the orbital rim deepens this contour over time. Thin under eye skin, which can be partly genetic and becomes more pronounced with age, allows the underlying vascular network to show through as a bluish tint. In some cases, true pigmentation from sun exposure or genetics contributes.

We use a cannula technique in most cases, which reduces the risk of bruising and allows more controlled, even placement than a needle. The amount placed is typically far less than clients expect.
Overfilling the tear trough produces puffiness, a grey or bluish cast, or an unnatural fullness that is distinctly worse than the original concern. Our approach is conservative by design: we place less than the maximum, assess the result, and only add more at a follow-up if needed and appropriate. The appointment takes approximately 30 to 45 minutes from arrival. Mild swelling is common for one to three days post-treatment.
For some clients, tear trough filler works best in combination with other treatments. Volume loss through the cheek often contributes to under eye hollowing by reducing support from below, and addressing both areas together produces a more harmonious result. Ideal candidates for tear trough filler are adults who have visible hollowing or shadowing beneath the eye that is not primarily caused by excess skin or fat prolapse.
Filler works well when there is a clear volume deficit along the tear trough creating a shadow. It is not the right tool for everyone. Significant under eye puffiness caused by prolapsed orbital fat, significant skin laxity, or dark circles caused primarily by pigmentation in the skin are not well served by filler and can be worsened by it.

Frequently Asked Questions About Under Eye Fillers
How long do tear trough filler results last?
Results typically last between 12 and 18 months for most clients, though this varies depending on the product used, the volume placed, and individual metabolism. The under eye area tends to hold filler well relative to higher-movement areas like the lips. Because we use conservative volumes, some clients return for a small top-up at the 12-month mark rather than a full retreat. We assess at each appointment and recommend based on what we see.
Is tear trough filler painful?
Topical numbing cream is applied before treatment, and the filler itself contains lidocaine, so discomfort during the procedure is minimal for most clients. The under eye area is sensitive and some clients notice brief pressure or stinging at the moment of injection, but this resolves immediately. The majority of clients describe the experience as more manageable than they anticipated.
What is the recovery time after under eye filler?
There is no true downtime, but mild swelling is common for one to three days after treatment and can occasionally last a little longer, particularly in clients who experience swelling easily. Bruising is possible but is less common with cannula technique. Most clients feel comfortable returning to work and social settings the day after treatment, using concealer if needed. We recommend avoiding vigorous exercise and significant sun exposure for 24 hours post-treatment.
Can tear trough filler be dissolved if I do not like the result?
Yes. Hyaluronic acid filler, which is what we use at Kontour, can be dissolved with an enzyme called hyaluronidase. If a result is not what you wanted, or if you experience an adverse effect, the filler can be safely reversed. This is one of the reasons we use hyaluronic acid products for the tear trough specifically: the reversibility provides an important safety margin in a delicate treatment area.
Will filler fix the dark circles under my eyes?
Puffiness after tear trough filler is the result of overfilling and is entirely avoidable with conservative technique. The under eye area requires very small volumes, and more is rarely better. Our approach is to place less than the maximum appropriate amount, assess the result with you, and plan any addition carefully at a follow-up. The outcome we aim for is a face that looks rested and natural, not a face where the treatment is visible. If at any point the result looks like too much, we can dissolve and reassess.
